Lakeland Florist Supply is a business to business wholesaler, importer and manufacturer of floral and floral related products. We assume that all purchases are intended to be resold or used in the general course of business. All new customers must be registered before purchasing either online or in the store. A copy of our membership form and the Minnesota state tax form are available for your convenience.

• Online Application - This is the first step if you wish to shop online. Please use this form to create an online account with us.
• Membership Application Form - All prospective customers of Lakeland Florist Supply will need to fill out and return this form to us.
• State Tax Form - Completion of the state tax form exempts approved accounts from state sales tax at the time of purchase.
• Credit Application Form - If you wish to apply for credit terms with Lakeland Florist Supply, you will need to fill out this form and return to us for evaluation.
